29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:   On or about 04/29/2014, in the parking lot of the Super 1 Foods located at 3747 Ambassador Caffery Parkway, Lafayette, LA 70503, the employer failed to ensure that two employees using a Genie aerial lift to paint a light pole were protected from falling to the pavement below.
