29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person: location: exterior of house on or about: 4/19/14 a) Employees were engaged in roofing work from a wall mounted bracket scaffold, that was not constructed in accordance with the design on the sticker that was placed on the 'American' Model 2200 Side Wall Staging Brackets. The spacing between the brackets was 17'5" and 19'1". There was no wood placed on the exterior of the sheathing. One of the brackets gave way, exposing the employees to a fall of 24 feet.
