29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a- Throughout Facility - Forklift training and evaluation was not provided to those employees who are required to operate forklift trucks. Training shall consist of among other things: - Formal training to include truck related topics and workplace related topics; - Practical training to include demonstrations by the trainer; - Evaluation of the operator's performance in the workplace. The employer must also certify that each employee was trained and evaluated. The certification is to include, but not be limited to, the name of the operator, the date of the training and evaluation, the name of the person providing the training or evaluation, etc. Condition noted on or about 5/13/14.
