29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) For employees working at 2638/2640 Anguilla Drive in Cape Coral, as observed on or about May 09, 2014. Employees were using an extension cord that was plugged into a permanent power outlet to provide power to tools utilized on the work site, that was missing the path to ground.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: a) For employees working at 2638/2640 Anguilla Drive in Cape Coral, as observed on or about May 09, 2014. Employees were using an extension cord that was plugged into a permanent power outlet to provide power to tools utilized on the work site, that was not protected from vehicular damage where it crossed the road.
