29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a) Unprotected sides of installed roof: On or about May 13, 2014, the employer did not implement and/or require use of a fall protection method while working on the roof which exposed employees to falls of 16-feet to the ground below. b) Leading edge of roof being installed: On or about May 13, 2014, the employer did not implement and/or require use of a fall protection method while working on the roof which exposed employees to falls of 16-feet to the ground below. c) Horizontal beams: On or about May 13, 2014, the employer did not implement and/or require use of a fall protection method while walking on the beams between the ladder and roof which exposed employees to falls of 16-feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a rigid support and a grasping device, such as a grab rail, provided to assist the employee in mounting and dismounting the ladder: a) Extension Ladder: On or about May 13, 2014, the employer did not ensure the ladder was secured at the top when the rails did not extend at least 3-feet above the landing surface which exposed employees to falls of 16-feet to the ground.
