29 CFR 1910.133(a)(1): Protective eye and face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:    a) Warehouse Area- The employer failed to require the use of protective eye and face protection, such as safety goggles and a face shield, for employees that were exposed to corrosive chemicals (sulfuric acid) while performing tasks such as handling and transporting leaking vehicle batteries.      No abatement certification or documentation is required for this item.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a) Warehouse Area- The employer did not provide suitable facilities for the quick drenching or flushing of the eyes for immediate emergency use where employees may be exposed to injurious corrosive materials while checking levels in and adding water to batteries on powered industrial vehicles.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    a) Warehouse Area- Woodworking table saws, such as the Craftsman Contractor Table Saw, were not equipped with a guard to protect the employees from the rotating blade.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
