29 CFR 1926.403(i)(2)(i)(A): Guarding of live parts: Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means: By location in a room, vault, or similar enclosure that is accessible only to qualified persons.  a. In the receiving area, a cabinet was not enclosed and protected from accidental contact by unqualified persons. Employees were exposed to contact with 480 Volt electrical current or arc flash/blast hazards.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.  a. On March 12, 2014, employees were utilizing an extension cord that had damaged external insulation, thereby exposing employees to a contact with electrical current hazard.  b. The extension cord plugged into the pillar near the main entrance that was used to charge the scissor lifts near the main entrance was damaged and the internal wires were exposed, thereby exposing employees to a contact with electrical current hazard.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.  a. By the south wall of the building a scissor lift was plugged into an extension cord in order to charge the lift. The scissor lifts cord and plug were missing the ground and thereby exposing employees to a contact with electrical current hazard.

29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a. In the receiving area a junction box did not have a cover and allowed employees access to live electrical wires. Employees were exposed to a contact hazard of greater than 50 Volts of electrical current.

29 CFR 1926.405(j)(1)(i): Live parts. Fixtures, lampholders, lamps, rosettes, and receptacles shall have no live parts normally exposed to employee contact. However, rosettes and cleat-type lampholders and receptacles located at least 8 feet (2.44 m) above the floor may have exposed parts.  a. On March 12, 2014, the employer was using a receptacle that did not have cover to energize an extension cord which in turn was used to charge scissor lifts. The employees were exposed to a contact with 120V electrical current hazard.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  a. On March 12, 2014, the employer failed to ensure inspections were conducted on the mobile scaffold scissor lifts prior to use. The scissor lifts had damaged electrical components that powered the control mechanism. Employees were exposed to a fall hazard from mechanical failure, and exposure to electrical contact.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.  a. Near the receiving area employees were exposed to a tripping hazard from a unprotected floor hole which was 10 inches by 10 inches and 2 inches deep.
