29 CFR 1910.242(b):  Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:   a)  Judson A. Smith Co. - On or about May 27, 2014, employees were exposed to flying particles and compressed air hazards from air nozzles used for cleaning small parts at the mobile pneumatic press table, where the dead end pressure exceeded 30 p.s.i.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

Smith Co. - On or about May 27, 2014, employees were exposed to shock hazards while performing work at the mobile pneumatic press table that had a relocatable power tap being used to power a light, rotary machine, and the power source for a pneumatic press. Smith Co. - Smith & Nephew Area - Employees working within close proximity to a live unguarded 110 volt junction box used to power a voltage step down controller for the DC Speed Controller, were exposed to electrical shock/burn hazards while operating the Rotary Machine on or about May 27, 2014.      Abatement certification required within 10 days after abatement date. Smith Co.  Plant wide - Employees use hazardous equipment such as Class 4 lasers to burn and weld metal, exposing employees to physical burn hazards to the skin and eyes.  The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified system or task evaluated, the person certifying that the evaluation had been performed, the date(s) of the hazard assessment, and, which identified the document as a certification of hazard assessment on or about May 27, 2014.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
