29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) Lot 228-Blue Ridge Drive, Pleasant Hills, PA: On or about May 16, 2014, an employee was observed framing a home with no fall protection provided with approximately a 20 foot fall hazard.

29 CFR 1926.503(c): The employer did not retrain affected employees  who already had been trained but demonstrated inadequate  understanding and skill required by paragraph (a) of this section:    (a) Jobsite- Lot 228- Blue Ridge Drive, Pleasant Hills, PA: On or about May 16, 2014, employees exposed to fall hazards of approximately 20 feet who had already been trained were not retrained to demonstrate adequate understanding of fall hazards.
