29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:   On or about May 15, 2014, the flexible power cord of the JLC Coffing ¼ ton electric hoist, located at the RP 506 P Press, was not fitted so that strain relief was provided to prevent the cord casing from pulling away from the cord plug.  The hoist power cord was plugged into an extension cord located between Press 401 and Press 402.

29 CFR 1910.184(e)(3)(i): A thorough periodic inspection of alloy steel chain slings was not made on a regular basis and/or were done at intervals greater than once every 12 months:   On and about May 15, 2014, the employer did not ensure a thorough periodic inspection was performed on a four spread alloy steel chain sling used to lift molds into the mold cleaning machine.
