29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:           a) Rough Mill:  On or about July 17, 2014, employee operating the Planer, Re-saw, Grinder, Gangrip and next to chain saw operation was exposed to continuous noise at 76.7% of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 85 dBA) during the 440 minute sampling period.  For a full eight hour work shift this exposure is equivalent to an average sound level of 88.0 dBA.  Exposure calculations include a zero increment for the 40 minutes not sampled.         b)  Rough Mill:  On or about July 17, 2014, employee helper operating the Planer, Re-saw, Grinder, Gangrip and chain saw were exposed to continuous noise at 103.6% of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 85 dBA) during the 435 minute sampling period.  For a full eight hour work shift this exposure is equivalent to an average sound level of 90.2 dBA.  Exposure calculations include a zero increment for the 45 minutes not sampled.

29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded in accordance with the specific requires of 1910.219:       a)  Rough Mill:  Employees were exposed to ingoing nip point hazard by an exposed belt from a broken cover on the Oliver Planer machine while feeding wood stock into and passing by the machine, SN 90028 located approximately 1 foot from the ground.       b) Rough Mill:  Employees were exposed to ingoing nip point hazard by an exposed belt from a broken cover with an opening measuring 1" by 4" on the Vecoplan-Anlagen machine while feeding scrap wood stock into it, SN 145801 located approximately 1 foot from the ground.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of electric equipment, such as parts that were broken, bent, cut or deteriorated by corrosion, chemical action, or overheating:         a)  Rough Mill:  On or about May 16, 2014, Employees were exposed to electrical hazards while feeding wood into the Vecoplan-Anlagen machine, SN 145801, and the cover of the motor housing was loose and not secured.         b)  Rough Mill:  On or about May 16, 2014, Employees were exposed to electrical hazards while feeding wood into the Vecoplan-Anlagen machine, SN 145801, and the conduit supplying the main power was broken.         c)  Rough Mill:  On or about May 16, 2014, Employees were exposed to amputation hazards while feeding wood stock into the Oliver Planer machine, SN 90028 the estop button at the main operation panel was broken and removed from the machine.
