29 CFR 1910.1030(g)(2)(i)(A): The employer did not train each employee with occupational exposure in accordance with the requirements of this section, and at the time of initial assignment to tasks where occupational exposure may take place:   Facility: When new employees were hired, and it was determined their job tasks entailed occupational exposure to bloodborne pathogens, the employer did not ensure these employees received training at the time of their initial assignment.

29 CFR 1910.1200(h)(1): Employers did provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area:  Facility: The employer did not conduct Hazard Communication training for employees at the time of their initial assignment and when new chemical hazards were introduced into the work area.  These include, but are not limited to, chemicals and products used in dental procedures,  and chemicals used in cleaning.
