29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):           (a)  Odyssey Environmental, 653 Jones Street, Lorain, Ohio:  On or about May 15, 2014, at the work site located at 1807 East 28th Street, Lorain, Ohio, the employer did not assess the workplace to determine what hazards were present or likely to be present when conducting tank cleaning activities.  A chemical was released to the atmosphere when several chemicals were combined in a vacuum truck.  Employees were at risk of serious injury from inhaling the released chemical.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:           (a)  Odyssey Environmental, 653 Jones Street, Lorain, Ohio:  On or about May 15, 2014, at the work site located at 1807 East 28th Street, Lorain, Ohio, the employer did not identify the respiratory hazards to which employees were exposed when several chemicals were mixed during cleaning of the tanks in the bloom caster and did not evaluate the impact the chemicals would have on the employee's health.  Employees were at risk of serious injury from exposure to an airborne chemical that was released when waste chemicals were mixed together.
