29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:    On or about May 19, 2014, at the above addressed site, an employee performing pressure cleaning work from the basket of an aerial lift approximately 15 feet above a lower level was exposed to a potential fall hazard while not wearing a harness with lanyard attached to the boom or basket of that lift.
