29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) shall be protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board;  On or about 05/19/2014, at the site, an employee using chicken ladder to stand on during a chimney repair work, was not protected from falling. The roof pitch was approximately 8:12. No other means of fall protection was used.

29 CFR 1926.501(c): When employees were exposed to falling objects the employer did not require employees to wear hard hats and did not implement measures to prevent objects from falling from higher levels as set forth in (c)(1)-(3) of this section:  At the site, the employee who was on the ground level passing masonry materials such as bricks and cement to the workers on scaffolds was not required to wear protective helmet.
