29 CFR 1926.453(b)(2)(v): A personal fall protection system shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  North side of the shopping center, an employee worked from inside of the bucket of an aerial lift truck, Ford model F350 Super Duty, without the use a personal fall  arrest system, exposing the employee to fall hazards of approximately 16 feet.
