(a) Jobsite, 519 Tripoli Street, Pittsburgh, PA:  On or about 05/19/14 the ladder jack scaffold, consisting of 2 aluminum ladders and an aluminum platform, was erected and used within 4 inches of the insulated lines that were less than 300 volts.  The insulated power line ran across the platform and behind the ladder on the right of the scaffold.  The employees had to climb over the power line to access the platform and work area.
