29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). Note:  Specify distances as indicated in table in paragraph (f)(6) of this section.    (a) Jobsite, 519 Tripoli Street, Pittsburgh, PA:  On or about 05/19/14 the ladder jack scaffold, consisting of 2 aluminum ladders and an aluminum platform, was erected and used within 4 inches of the insulated lines that were less than 300 volts.  The insulated power line ran across the platform and behind the ladder on the right of the scaffold.  The employees had to climb over the power line to access the platform and work area.

29 CFR 1926.452(k)(1): Platforms of ladder jack scaffolds exceeded a height of 20 feet (6.1 m):      (a) Jobsite, 519 Tripoli Street, Pittsburgh, PA:  On or about 05/19/14 the platform on the ladder jack scaffold was installed at approximately 22 feet above the ground.  The employees worked from the platform removing debris from the roof and tossing it to the ground and to access the work area on the roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501:    (a) Jobsite, 519 Tripoli Street, Pittsburgh, PA:  On or about 05/19/14 employees engaged in residential construction activities, working on a roof approximately 24 feet above the ground were not protected from falling by the use of guardrail systems, personal fall arrest systems, or safety nets.
