Location: Worksite Employees were working on top of a trailer located 12 feet above the ground and were not protected from falling by guardrails, safety nets, or personal fall arrest systems.

29 CFR 1926.62(i)(2)(ii): The employer did not ensure that change areas were equipped with separate storage facilities for protective work clothing and equipment and for street clothes, that would prevent cross-contamination: Location: Worksite The employer did not have an adequate number of lockers in the decontamination trailer's clean room where employees' street clothes were supposed to be stored. There was no storage area on the dirty side of the decontamination trailer where contaminated clothing was collected for laundering.
