29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions.    a) On or about May 20, 2014 employee operating a Boom forklift was not wearing the seatbelt equipped while exposed to hazardous conditions that could result in forklift turning over and crushing the operator.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    a) On or about May 20, 2014 employees were using a portable metal ladder and wood ladders that did not extend three feet above the upper landing.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)    a) On or about May 20, 2014 employees were observed using a portable metal ladder that was not secured above the upper landing.    b) On or about May 20, 2014 employees had used two wooden home made ladders that was not secured above the upper landing.
