29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) Southeast roof edge above Entrance 1, 1403 East Third Street, Nescopeck, PA - Employees engaged in extending the roof span above Entrance 1 stood on a board being utilized as a scaffold and were exposed to a fall hazard of sixteen-foot, eight-inches (16'8") above a concrete loading dock. The employer did not ensure that a personal fall arrest system or guardrail system was utilized to protect employees from falling, as observed on or about May 21, 2014. In accordance with 29 CFR 1903.19(c) and (d), abatement certification and documentation is required for this violation.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Southeast roof edge above Entrance 1, 1403 East Third Street, Nescopeck, PA - Employees engaged in extending the roof span above Entrance 1 were exposed to a fall hazard that ranged from nine-foot, three-inches (9'3") to sixteen-foot, eight-inches (16'8") above a concrete loading dock. The employer did not ensure that fall protection systems were utilized to protect employees from falling, as observed on or about May 21, 2014. b) Northeast roof edge, 1403 East Third Street, Nescopeck, PA - An employee engaged in removing the northeast corner of the roof of a building was exposed to a fall hazard that ranged from twelve-foot, one-inch (12'1") to twenty four-foot, eight-inches (24'8") above ground level. The employer did not ensure that fall protection systems were utilized to protect the employee from falling, as observed on or about May 21, 2014. In accordance with 29 CFR 1903.19(c) and (d), abatement certification and documentation is required for this violation.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) Southeast roof edge above Entrance 1, 1403 East Third Street, Nescopeck, PA - Employees used a Vulcan 17-foot Multi-task Ladder Model ES-17TIA-CP that did not have side rails that extended at least three-foot (3') above the roof eave and that was not secured when mounting and dismounting from the ladder onto a roof. The employees were exposed to a fall hazard of nine-foot, three-inches (9'3"), as observed on or about May 21, 2014. b) Northeast roof edge, 1403 East Third Street, Nescopeck, PA - Employees used a Vulcan 17-foot Multi-task Ladder Model ES-17TIA-CP that did not have side rails that extended at least three-foot (3') above the roof eave and that was not secured when mounting and dismounting from the ladder onto a roof.
