29 CFR 1926.501(b)(13): The employer did not ensure that each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:     a) West County Roofing, L.L.C. @ 1164 Trail Drive, Chesterfield, MO 63005: On May 21, 2014, the employer did not ensure that employees were protected against fall hazards. Three employees conducted roofing work on a two-story, residential home without the use of a guardrails or personal fall arrest systems. An employee installed shingles on an 8 in 12 pitch portion of the roof, another conducted tear off work on a 10 in 12 pitch portion of the roof, and the last employee prepared to install roofing felt on the 8 in 12 pitch portion of the roof. This condition exposed employees to a fall hazard of approximately 11 feet.     West County Roofing, L.L.C. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315653204, citation number 1, item number 2 and was affirmed as a final order on March 12, 2012, with respect to a workplace located at 1137 Tree Top Village Drive, Ballwin, MO, 63021.
