29 CFR 1926.453(b)(2)(iv):  Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:   (a). At a construction site along Hwy 50 in Orlando, an employee elevated in the basket of an aerial lift with no lanyard attached to the boom or basket, was standing on the guardrails and not standing firmly on the floor of the basket exposing himself to an 18-foot fall hazard, on or about May 23, 2014.

29 CFR 1926.453(b)(2)(v):  A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a). At the Wendy's construction site along Hwy 50 in Orlando, the employee installing exterior finishing did not have his lanyard attached to the boom or basket while elevated exposing him to a 18-foot fall hazard, on or about May 23, 2014.
