29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:   (a) On or about May 28, 2014, at the facility, the employee coating military collars in acetone was not wearing any eye protection. Acceptable eye protection would be the use of chemical splash goggles or a face shield.  (b) On or about May 28, 2014, at the facility, the employee coating military collars in acetone was wearing nitrile gloves. Nitrile gloves are not recommended for protection against skin contact with acetone. The recommended gloves to prevent skin contact with acetone would be gloves made of butyl rubber.

29 CFR 1910.124(j)(3): Periodic inspections of all dipping and coating equipment, including covers, drains, overflow piping, and electrical and fire-extinguishing systems were not conducted, and deficiencies were not promptly corrected:     (a) On or about May 28, 2014, in the facility, the employer did not inspect the interior and exterior piping system, the outside acetone holding tank, and the brackets holding the outside acetone storage tank off the ground. The piping, acetone storage tank and the brackets holding the storage tank showed evidence of external corrosion.

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:  (a) On or about May 23, 2014, in the break area, brown paint chips containing .3252% lead were found on the floor in the area adjacent to the boiler room that was being used as a break area. The paint was flaking off the ceiling in this area.  (b) On or about May 23, 2014, in the storage area, beige paint chips containing .2630% lead were found on the shelves, in the water fountain and on the floor in the storage area next to the office.  (c) On or about May 23, 2014, in the cutting room, white paint chips containing .2162% lead were found on the cutting tables.
