29 CFR 1910.184(c)(6): Slings were not securely attached to their loads: (a) On or about May 20, 2014 the inspection revealed slings that were not securely attached to their load. Located at the overhead crane where chain slings were improperly secured to a water tank. The crane was located behind the fleet maintenance shop at the Scott Street compound, 75 SE Scott Street in Bend, Oregon.

29 CFR 1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer: (a) On or about May 20, 2014 the inspection revealed that chain slings were not inspected by a competent person to ensure that they were fastened to the load securely before being used. Slings were attached to a water tank that was located behind the fleet maintenance shop at the Scott Street compound in Bend, Oregon.

29 CFR 1910.1001(d)(2)(i): Each employer did not perform initial monitoring of employees who were, or could reasonably be expected to be exposed to airborne concentrations at or above the action level and/or excursion limits: (a) On or about May 20, 2014 the inspection revealed that the employer did not perform initial monitoring of employees who could reasonably be expected to be exposed to airborne concentrations of asbestos. Located in the vehicle maintenance and repair shop at 74 SE Scott Street in Bend, Oregon employees are potentially exposed to airborne asbestos fibers when grinding on brake drums.
