29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  (a) The employer failed to conduct inspections of the workplace to ensure that employees were not exposed to hazardous situations.  This exposed employees to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  The employer failed to ensure that each employee conducting roofing work at 8425 West Mill Road in Milwaukee, Wisconsin, were protected from falling to the lower level by the use of a fall protection system.  This exposed employees to a fall hazard of approximately 18.4 feet.
