29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  (a). Worksite located at 5261 Antioch Road, Kansas City, Missouri- Employees  were exposed to overhead hazards from elevated materials and employees and the employees were not protected from impact  by the use of any means of head protection.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:   (a). Worksite located at 5261 Antioch Road, Kansas City,  Missouri- Employees were exposed to impalement hazards in that rebar was not capped and was located near and below elevated employees on the scaffolding and walking near the scaffolding.
