29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  (a) Throughout facility: There were no machine specific lockout tagout procedures for machines such as, the Micropit Instrument Panel Assembly Line, and the Cooling Module Assembly Bolster and Main Line, as there were no machine shut down, and start up procedures with legible instructions.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  (a) Maintenance Shop: Employees were exposed to electrical hazards as the flexible cord for the Milermatic 251 Welder, did not have adequate strain relief as the outer insulation had pulled back next to the plug-in exposing interior wiring.
