OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards:  a.  On or about May 28, 2014, in the old engine room, the employer did not ensure a safety latch was attached to the hook and the hook was not removed from service, exposing employees to crushing and struck-by hazards.

On or about May 28, 2014, in maintenance bay 2 on the east wall, the lid on the parts washer was not closed when the parts washer was not in use, employees were exposed to hazards associated with combustible materials.      b.  On or about May 28, 2014, in maintenance bay 4 on the south wall, the lid on the parts washer was not closed when the parts washer was not in use, employees were exposed to hazards associated with combustible materials.

On or about May 28, 2014, in the tire changing bay, the in-line pressure gage installed on the tire restraint cage was by-passed and disabled, employees were inflating tires by reaching into the tire restraint cage, exposing employees to crushing and struck-by hazards.
