29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):  a)  In the Shred Room, three Shred Room employees were exposed to carbon dioxide at levels exceeding 25000 ppm on 06/06/2014, which over a period of approximately 100 minutes would expose employees to carbon dioxide above the PEL of 5000 ppm.  Controls in these circumstances may include, but are not limited to, the following:  1)  Introduce fresh air into the Shred Room to reduce carbon dioxide levels to below the PEL.  2)  Install equipment or controls on the bagging machine to meter the amount of carbon dioxide and nitrogen being injected into individual bags as opposed to a constant flow of the two gases.  3)  Reduce the amount of time employees run 5 lb bags in a day/shift to ensure carbon dioxide levels do not concentrate to levels that would exceed the PEL.
