(Model:  9026630) toggle switches were used as an energy isolating device.  This exposed employees to accidental / inadvertent restart of equipment.     (b) The Sebright Products Inc. compactor (S/N:  11 08 10637) selector switch was used as an energy isolating device.  This exposed employees to accidental / inadvertent restart of equipment.
