29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation. a) North face of building, 83 S.R 93, West Hazleton, PA - The base plates on the southern edge of the third buck of a fabricated frame scaffold system were placed on wooden planks that had visible damage such as, but not limited to splits, checks, and rotted wood. Employees were exposed to a fall hazard of twenty three-foot, six-inches (23'6").

29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold was not protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway. a) North face of building, 83 S.R 93, West Hazleton, PA - The walkway on the third buck between the top of the access ladder from the second buck and the bottom of the access ladder to the fourth buck was not protected on either side by an adequate guardrail system. Employees were exposed to a fall hazard of nineteen-foot (19').

29 CFR 1926.452(c)(5)(iii): Brackets used to support cantilevered loads were not used only to support personnel. The scaffold was not designed for other loads by a qualified engineer and built to withstand the tipping forces caused by those other loads being placed on the bracket-supported section of the scaffold. a) North face of building, 83 S.R 93, West Hazleton, PA - A fabricated frame scaffold system that was not designed by a qualified engineer had side brackets on the second level that were used to support the cantilevered third and fourth levels. Employees were exposed to a fall hazard of twenty three-foot, six-inches (23'6").
