29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) Matt Sours DBA MBS Contracting on or about May 30, 2014, at the site address of 709 Napoleon St. Fremont, OH: The employer failed to ensure employees were protected from a fall while working from a ladder jack scaffold. Employees were exposed to a near 19 ft. fall to the level below while working from the platform during roofing operations of a residential structure.
