Matheney dba Elite Roofing & Siding, 829 Market Street, Meadville, PA  16335, on or about May 30, 2014 and times prior thereto:  Employees walking on a lower roof were exposed to potential injury from bundles of shingles that had the potential to slide down from the upper roof and strike the employees below.  The employees were provided with head protection, however the use of head protection was not utilized.     B.)   Dusty A. Matheney dba Elite Roofing & Siding, 829 Market Street, Meadville, PA  16335, on or about May 30, 2014 and times prior thereto:  Safety glasses are provided and used for operations including, but not limited to, cutting and nail gun operations.  Safety glasses were not being utilized by personnel who were tearing up shingles.
