29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:  On or about June 2, 2014, at 4733 Hwy 46, West, New Braunfels, TX, the employer failed to ensure the employee was protected from a fall hazard from a metal work platform attached with straps to the rough terrain Track forklift, model 6036.  The basket and its attachment means were not designed in accordance with the guidance set forth by the manufacturer. The employee was exposed to a fall hazard of approximately 12 feet and was exposed to being struck by the metal basket in the event it fell off the forks.
