29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a). At 241 Meadow Beauty Terrace in Sanford, Florida, imminent danger existed in the form of serious injury where employees were exposed to an 11-foot fall hazard in that no fall protection was utilized while installing a solar system on a 5:12 pitched roof, on or about May 28, 2014.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet  (.9 m) above the upper landing surface to which the ladders were used to gain access.  (a) At 241 Meadow Beauty Terrace in Sanford, Florida, employees were using an extension ladder that was positioned only 1 and 1/2 feet above the roof edge which exposed them to a fall hazard of 11 feet in that during access and egress from the roof, on or about May 28, 2014.
