29 CFR 1926.404(f)(1)(iv)(C): AC premises wiring system was not grounded where the system is nominally rated 240/120 volt, 3 phase, 4 wire in which the midpoint of one phase is used as a circuit conductor:(a) Behind the pump station and in front of the water tank.  Exterior electrical panel with multiple outlets that was covered with a garbage bag.  Employees exposed to electrical hazards for not having waterproof covers on the outlets while in use.  Jobsite:  Employees on site exposed to 120/240 volt temporary service panel that was not grounded.
