29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions.  (a)  Outside building 243:  On or about April 16, 2014 and times prior thereto, the employee operating the Zaxis 120 was not wearing a safety belt.  This exposed employee to a struck-by hazard if the machine were to tip ejecting the employee from the cab.
