Employees have not been evaluated/re-trained after they have been operating for 3 years.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:   The employer is failing to protect employees from laceration hazards by not properly guarding equipment.  The work rest on the bench grinder in the maintenance shop was not adjusted adjacent to the wheel to the required 1/8" gap.  The gap measured at approximately 3/4".

The grinder in question had no tongue guard on it at the time of the inspection.
