29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a)  Employees were exposed to amputation hazards while replacing the pressure switch on Press #11 (Cincinnati Plastic Injection Mold, Model #300T and Serial #4053A21/85-9) and the employer did not ensure that lockout/tagout was being utilized.

29 CFR 1910.179(j)(2): The following items were not be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections:  (a)  Employees were working with the overhead crane and the employer did not ensure that frequent (monthly) crane inspections were being done on items such as the operating mechanisms, hooks, hoisting chains and the hydraulic system.
