OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards associated with improper automobile lift operations: (a) On or about June 3, 2014, and at times prior, Tri County GM, Inc. employees were exposed to crushing and struck-by hazards while working around automotive lifts as follows: 1 - Lift inspection certificates and checklists were not maintained as required by ANSI/ALI ALOIM 2008. 2 - Lifts were not inspected daily during use to ensure that they were properly maintained and functional. 3 - Lift #7 showed deformation and wire strain on the over-travel shut off device. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

Employees around work bench #6 were exposed to contact with electrical hazards. A fluorescent light fixture was observed with severe strain on its cord at the point where the cord meets the plug.
