29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:   Employees performing siding tasks from a fabricated scaffold at 234 Gage, Topeka, Kansas were exposed to fall hazards of twenty (20) feet and greater to the ground below. The scaffold, attached to the JCB Model # 506CHL telescopic handler was not designed by a qualified person.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold more than 10 feet (3.1m) above a lower level, not otherwise specified in paragraphs (g)(1)(I) through (g)(1)(vi) of this section, was not protected from falling to that lower level by use of personal fall arrest systems or guardrail systems meeting the requirement of paragraph (g)(4) of this section:   Employees performing siding tasks from a fabricated scaffold at 234 Gage, Topeka, Kansas were exposed to fall hazards of twenty (20) feet and greater to the ground below. The scaffold, attached to the JCB Model # 506CHL telescopic handler had the following deficiencies:        (a)    a missing top rail,     (b)    missing mid-rails;     (c)    no toe boards.
