29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.    Jobsite: Employees were working on a fabricated frame scaffold platform, which was not fully planked on each working level and exposed to a fall hazard.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.    Jobsite: The ends of scaffold planks/platforms, from that the employees were working, extended over the supports far more than 18 inches, and no other safe means were utilized to prevent the planks/platforms from tipping.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.    Jobsite:  The fabricated frame scaffold platform, from which employees were working at a height more than 18 feet above the ground was not equipped with appropriate and safe means of access.  On or about June 04, 2014, one of employees fell to the ground while attempting to climb down the frames of the scaffold and sustained serious injuries from the incident.

Jobsite:   The employees, working on the platforms of the fabricated metal frame and cantilevered end supported scaffolds were not protected from the fall hazards by the use of appropriate fall protection devices and/or systems.
