29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards. Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  a) Warehouse Office Roof, 625 South Main Street, Wilkes-Barre, PA - Employees placed and removed empty boxes as needed from the open-sided eastern edge of the Warehouse Office roof that was not guarded with standard railings. Employees were exposed to an eight-foot, three-inch (8'3") fall hazard from the roof to the concrete Warehouse floor, on or about June 4, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.    a) Along Northwest Wall, Box Room, 625 South Main Street, Wilkes-Barre, PA - Employees used a power strip (relocatable power tap-RPT) attached to an extension cord (flexible power cord) to provide electrical power to labeling equipment. Employees were exposed to contact with an electrical current hazard, as observed on or about June 4, 2014.  c) North Corner, Box Room, 625 South Main Street, Wilkes-Barre, PA - Employees used a 3-way outlet adapter (current tap) that was not securely attached to an extension cord (flexible power cord) to provide electrical power to labeling equipment. Employees were exposed to contact with an electrical current hazard, as observed on or about June 4, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The hazard was corrected during the inspection.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch  a) Repair Shop, 625 South Main Street, Wilkes-Barre, PA - Employees operated the Central Machinery Bench Grinder Stock No. S5808 (Serial No. 9447) that had an opening between the side guard and outer periphery of the abrasive wheel on the left that was two and one-fourth inches (2 1/4") and an opening between the side guard and outer periphery of the abrasive wheel on the right that was seven-eighths of an inch (7/8"). Employees were exposed to struck-by hazards on or about June 4, 2014.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
