29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  On or about June 4, 2014, an employee removing shingles from a 8 feet high roof, was not protected by fall protection, i.e.; guardrails, safety nets, or personal fall protection.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  a.  On or about June 4, 2014, an employee did not have a ladder and/or stairway to access the roof he was removing shingles from over the resident laundry, exposing the employee to an 8 feet fall hazard.
