OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  Well site:  On or about June 5, 2014, one of the employees operating service rig during the repair of an oil well worked from the derrick mounted rod basket.  The emergency escape system (Geronimo Line) provided for the employee's emergency egress use had its line obstructed and its ground anchorage was insufficient.  This exposed the employee to the hazard of falling.
