29 CFR 1915.160(d): Before using a positioning device system, employees were not trained in the application limits, proper hook-up, anchoring and tie-off techniques, methods of use, inspection, and storage of positioning device systems. On or about 06/13/2014, the employer did not affectively train the use of the alternate safety device in lieu of the fixed ladder safety device to all affected employees.
