29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: (a) 6462 Carlisle Pike, Mechanicsburg Pennsylvania - Employees worked approximately 8 feet 3 inches above the ground installing plywood. The employer did not ensure that adequate fall protection was used, on or about June 9, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The employer did not insure that the fork were lowered when the operated was no longer in attendance of the powered industrial truck, on or about June 9, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
