29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to travel with the load trailing whenever the load obstructed forward view:  (a)  On or about April 16, 2014; a forklift operator traveled forward with three (3) totes high obstructing the view and made contact with additional totes on another forklift exposing employees to struck-by hazards.

29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics shall be provided to the operator when:  The operator has been involved in an accident or near-miss:  (a)  On or about April 16, 2014; two forklifts with totes on them were involved in a near miss in which refresher training was not conducted exposing employees to struck-by / caught-in hazards.  (b)  On or about May 27, 2014; a forklift was utilized to move a cardboard box which was collapsing and an employee's arm was pinned between the cardboard box and tote.  Refresher training was not conducted exposing employees to struck-by / caught-in hazards.
