29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) In the sanding area: On June 10, 2014 and at times prior to; the employer failed to develop specific lock-out, tag-out (LOTO) procedures for each machine such as, but not limited to the wide belt sander that uses 480 VAC and 85 psi of pneumatic pressure, exposing employees to caught in hazards during maintenance that is conducted at least 4 times each week. (b) In the saw area: On June 10, 2014 and at times prior to; the employer failed to develop specific lock-out, tag-out (LOTO) procedures for each machine such as, but not limited to the Gang Rip Saw that is operated by 480 VAC and 105 psi of pneumatic pressure, exposing employees to caught in hazards during maintenance for tool change at least one time each day.
