29 CFR 1926.102(a)(2): Eye and face protection equipment required by this Part did not meet the requirements specified in American National Standards Institute, Z87.1-1968, Practice for Occupational and Educational Eye and Face Protection:     Inside the structure: The employer did not provide approved goggles to protect employees eyes from exposure to Kelly Moore 1711 Alkyd Primer and 1700 Kel Guard Enamel paints. The material safety data sheets for the paints required approved goggles. The employer provided thin, clear plastic Ninja glasses with open slits for the eyes. There were no ANSI markings on the Ninja glasses. Employees were exposed to the hazards of their eyes contacting the paints.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws:  Inside the main structure: The insulation was pulled away from the plug on an extension cord that powered a 110 VAC Graco 390 paint sprayer, exposing employees that plugged in the sprayer to paint the columns of structure to the hazard of contacting electricity.
